The local court will only flog you certain types of forms. You should check your state's judicial council / court website.

But, as I recall, there is no standard form (i.e. a judicial council form) to set aside a non-attendance. Check out Matthew Bender's Pleadings and Practice books at your local law library (you cannot find it free online) and it will have sample.
